How do I make an appointment at the Los Angeles Neuromuscular Center (LANMC)?

You may make an appointment by calling 310.652.0010  Monday -Friday from 8:00am to 4:30pm, or you can send an email anytime to drmahajan@attunehealth.com. Check your health insurance policy to see if you need a physician referral.

What should I bring to my appointment?

-If you have been seen my doctors regarding your neurological symptoms in the past, it is most helpful if you bring a health history and records of your consultations, if available.

-Bring a list of your medication and mention any allergies you may have: this list should include all over-the-counter medications, home remedies, and herbal medications including tea, vitamins and weight gain or loss products such as shakes, pills or bars. Sometimes they can be dangerous when you take them with other medications.

-Know what medications you are taking, why you are taking them, and potential side effects. Let the doctor and nurse know of any allergies and type of reaction or side affects you have. Also be sure to ask questions about the medications you are prescribed during your appointment.

You may check with or your insurance company to determine whether pre-authorization for your appointment or any special testing is needed prior to your visit. You may also want to ask if you are responsible for any co-payment. Please bring your insurance card with you to your appointment.
